1. The Federal Norms and Rules in the field of industrial safety 'Rules for the Safe Conduct of Gas-Hazardous, Hot and Repair Work' (hereinafter - the Rules) have been developed in accordance with Federal Law No. 116-FZ of 21 July 1997 'On Industrial Safety of Hazardous Production Facilities' (hereinafter - the Federal Law 'On Industrial Safety of Hazardous Production Facilities') (Collected Legislation of the Russian Federation, 1997, No. 30, Article 3588; 2018, No. 31, Article 4860) and with the Regulations on the Federal Environmental, Industrial and Nuclear Supervision Service (Rostechnadzor) approved by Government Resolution of the Russian Federation No. 401 of 30 July 2004 (Collected Legislation of the Russian Federation, 2004, No. 32, Article 3348; 2020, No. 27, Article 4248).
2. The Rules establish requirements for the organisation and procedure for the safe conduct of gas-hazardous, hot and repair work (including earthworks) at hazardous production facilities at which the hazardous substances listed in paragraph 1 of Appendix No. 1 to the Federal Law 'On Industrial Safety of Hazardous Production Facilities' are obtained, used, processed, formed, stored, transported or destroyed, to prevent gas contamination of the air in the working area by flammable and toxic gases and the formation of explosive gas mixtures capable of causing explosions and fires.
3. The requirements of the Rules apply to gas-hazardous, hot and repair work, including earthworks, at the hazardous production facilities specified in paragraph 2 of these Rules, performed by employees of operating organisations and also by contractor organisations where a service contract concluded in accordance with the legislation of the Russian Federation is in place.
4. The Rules do not apply to: (a) construction, installation and commissioning work during the construction or reconstruction of capital construction facilities on a designated part of the territory of hazardous production facilities in operation, access to the site of which is restricted by solid enclosing structures made of precast reinforced concrete, metal or other fire-resistant materials not less than 2.2 metres in height and to which the ingress of vapours and gases of hazardous substances from operating production is excluded; (b) gas-hazardous, hot and repair work at hazardous production facilities: of the mining and metallurgical industry; of the heat and electric power industry, with the exception of fuel handling areas, fuel oil storage areas and diesel power plant areas at which the flammable substances listed in sub-clause (c) of paragraph 1 of Appendix No. 1 to the Federal Law 'On Industrial Safety of Hazardous Production Facilities' are stored and transported; of gas distribution and gas consumption (external and internal gas pipelines, gas-using equipment) falling within the scope of technical regulation of the Technical Regulations on the Safety of Gas Distribution and Gas Consumption Networks approved by Government Resolution of the Russian Federation No. 870 of 29 October 2010 (Collected Legislation of the Russian Federation, 2010, No. 45, Article 5853; 2018, No. 52, Article 8288); at facilities using liquefied petroleum gases (gas filling stations; gas filling points; intermediate stores of cylinders containing liquefied petroleum gas; tank installations, including external and internal gas pipelines and the technical devices of facilities using liquefied petroleum gases as fuel); at stationary motor vehicle gas filling stations, motor vehicle gas compressor filling stations and cryogenic filling stations for gas motor fuel; at linear facilities of trunk pipeline transport of oil, gas and gas condensate and of oil product pipelines, and also at facilities for the drilling and production of oil, gas and gas condensate and for the development of oil, gas and gas condensate fields, provided that requirements for the organisation and safe conduct of gas-hazardous, hot and repair work are established in regulatory legal acts in the field of industrial safety for those facilities, and in the absence of such acts - in the documents of the operating organisation (standards, instructions, regulations, process flow charts) agreed with the professional emergency rescue service (unit) and developed with regard to the requirements of these Rules and to the operating conditions of the hazardous production facilities.
5. A mandatory requirement for managers and engineering and technical personnel of operating and contractor organisations responsible for the preparation and conduct of gas-hazardous, hot and repair work is completion of certification in the field of industrial safety to an extent corresponding to their job duties. Certification of managers and engineering and technical personnel of operating and contractor organisations responsible for the preparation and conduct of gas-hazardous, hot and repair work is carried out in accordance with the requirements of the Regulations on Certification in the Field of Industrial Safety, on Matters of the Safety of Hydraulic Structures and of Safety in the Electric Power Industry approved by Government Resolution of the Russian Federation No. 1365 of 25 October 2019 (Collected Legislation of the Russian Federation, 2019, No. 44, Article 6204).
6. Persons not younger than 18 years of age who have undergone a medical examination in accordance with the requirements of the legislation of the Russian Federation and who have been trained in the techniques and methods of performing the work are admitted to gas-hazardous, hot and repair work.
7. Operating organisations shall develop documents (standards, instructions, regulations, process flow charts) that refine and specify the requirements for the organisation and conduct of gas-hazardous, hot and repair work at hazardous production facilities, including the procedure for briefing the responsible persons and the persons performing that work, with regard to the fire and explosion hazard characteristics and the physicochemical properties of the hazardous substances handled in the process.
8. When performing gas-hazardous, hot and repair work at hazardous production facilities, contractor organisations shall be guided by these Rules and by the documents (standards, instructions, regulations, process flow charts) of the operating organisation. The timing of that work is agreed by the head of the operating organisation or an authorised deputy, or by the head of a branch or an authorised deputy, with regard to the particular features of the use of an electronic signature established by paragraphs 24, 82 and 130 of these Rules.
9. Gas-hazardous work means work involving internal inspection, cleaning, repair or depressurisation of process equipment and service lines, the fitting and removal of blanks on equipment and pipelines, and also work inside vessels (apparatus, drying drums, process and drying furnaces, reactors, tanks, tankers, and also collectors, tunnels, wells, pits, trenches (one metre or more in depth) and other similar places), during which the release into the working area of fire- and explosion-hazardous or harmful vapours, gases and other substances capable of causing an explosion or ignition occurs or is not excluded, and also work where the oxygen content in the working area is insufficient (volume fraction below twenty per cent).
10. Gas-hazardous work, including work involving the presence of people inside apparatus, vessels and other equipment, shall be carried out only in those cases where it cannot be mechanised, automated or performed without the direct participation of people.
11. The operating organisation shall take measures to reduce the amount of gas-hazardous work and to raise the level of its safety by introducing mechanised and (or) automated methods of performing it. The choice of methods and means of cleaning process equipment and service lines of the fire- and explosion-hazardous and chemically hazardous substances used and formed, and also of the means of shutting down and stopping individual units and apparatus to prepare process equipment for repair work, shall be determined by the design documentation.
12. Depending on the degree of hazard, gas-hazardous work is divided into groups: I - work carried out with a permit to work for gas-hazardous work; II - work carried out without a permit to work for gas-hazardous work, but with mandatory registration of such work before it starts in the logbook of gas-hazardous work carried out without a permit to work. Work to contain and eliminate the consequences of accidents is performed without a permit to work for gas-hazardous work until the direct threat of harm to the life, health or property of other persons and to the environment is removed, and is carried out in accordance with the action plan for the containment and elimination of the consequences of accidents.
13. In an operating organisation (branch of an organisation) a list of gas-hazardous work shall be developed for each structural subdivision (production unit, shop, department, installation, section) (a recommended form is given in Appendix No. 1 to these Rules).
14. The list of gas-hazardous work shall specify: the structural subdivision of the organisation (production unit, shop, department, installation, section); the location and nature of the work; the harmful and hazardous production factors possible during its performance; the category of persons performing that work (employees of the operating or of a contractor organisation), and also of the personnel of the organisation's own professional emergency rescue service (unit) (hereinafter - the PASS(F)) certified to carry out gas rescue operations under the procedure established by the Regulations on the Certification of Emergency Rescue Services, Emergency Rescue Units, Rescuers and Citizens Acquiring the Status of Rescuer approved by Government Resolution of the Russian Federation No. 1091 of 22 December 2011 (hereinafter - the Regulations on the Certification of Emergency Rescue Services) (Collected Legislation of the Russian Federation, 2012, No. 2, Article 280; 2018, No. 19, Article 2741), or of a PASS(F) with which a service contract has been concluded and which is engaged where necessary to take part in that work; the principal actions ensuring the safety of the work performed.
15. The list of gas-hazardous work is developed by the head of the structural subdivision (production unit, shop, department, installation, section) and is agreed with the organisation's own PASS(F) certified to carry out gas rescue operations, or with a PASS(F) with which a service contract has been concluded, and with the production control service for compliance with industrial safety requirements or with the persons appointed responsible for exercising production control. The head of the operating organisation determines, by internal documents of the organisation, the procedure for agreement and the structural subdivisions on which powers to agree the list of gas-hazardous work are conferred, with regard to the requirements of these Rules.
16. The list of gas-hazardous work shall be approved by the head of the operating organisation or an authorised deputy, or by the head of a branch or an authorised deputy. The list of gas-hazardous work shall be reviewed and re-approved within the periods determined by the internal documents of the operating organisation, but at least once a year, and also when the process or the process flow diagram of the production changes. Lists of gas-hazardous work shall be kept in the structural subdivisions and in the subdivisions that agree permits to work for gas-hazardous work. Lists of gas-hazardous work may be kept in the electronic database of the operating organisation (branch of the organisation). The list of persons of the operating organisation responsible for the preparation and conduct of gas-hazardous work shall be approved by the head of the operating organisation or an authorised deputy, or by the head of a branch of the organisation or an authorised deputy. The list of persons responsible for the safe conduct of gas-hazardous work of a contractor organisation shall be approved by the head of the contractor organisation and sent to the operating organisation (branch of the organisation) at whose facility the work is to be carried out. The lists of responsible persons of the operating and contractor organisations may be kept in the electronic database of the operating organisation (branch of the organisation).
17. Where a need arises to carry out gas-hazardous work that is not included in the approved list of gas-hazardous work, such work shall be performed under a permit to work for gas-hazardous work in accordance with the requirements of these Rules, and shall subsequently be entered in the list of gas-hazardous work within ten days.
18. For group I gas-hazardous work a permit to work for gas-hazardous work is issued in two copies (a recommended form is given in Appendix No. 2 to these Rules), providing for the development and subsequent implementation of a set of actions for the preparation and safe conduct of the work. Gas-hazardous work performed under a permit to work for gas-hazardous work shall be carried out on working days during daylight hours (the day shift). Gas-hazardous work at night or during a thunderstorm shall not be permitted. With the written permission of the person who approved the permit to work for gas-hazardous work, urgent gas-hazardous work may be carried out during the hours of darkness (the evening and night shifts) and also on days off and public holidays in the presence of a representative of the PASS(F) or of certified representatives of emergency rescue services (units) for facilities of trunk pipeline transport and underground gas storage facilities, with notification of the persons who agreed the permit to work. Urgent gas-hazardous work means work of the highest priority and of an immediate nature aimed at preventing accidents at a hazardous production facility and a threat of harm to the life and health of the employees of the hazardous production facility. The permit to work for gas-hazardous work shall provide for additional actions to ensure the safe conduct of the work that take account of the conditions of its performance during the hours of darkness, including: the availability of main and emergency lighting and the installation of additional lighting equipment (where necessary) of explosion-protected design; the provision to the personnel engaged in gas-hazardous work during the hours of darkness of individual lighting devices of explosion-protected design (personal torches or portable electric lamps with batteries), and also of serviceable means of communication, equipment for monitoring the air environment and personal protective equipment corresponding to the nature of the possible hazard; the performance of the work in the constant presence of the person responsible for carrying out the work. Permits to work for gas-hazardous work shall be recorded by the organisation's own PASS(F) established by the organisation, or by another service on which those powers are conferred in accordance with the documents of the operating organisation (branch of the organisation), in the register of permits to work for gas-hazardous work, with the assignment of the next sequential number. The register of permits to work for gas-hazardous work shall be numbered, laced and sealed with a stamp (where one is held); its retention period is not less than six months from the day it is completed. The register of permits to work for gas-hazardous work may be kept in the form of an electronic document provided that the safekeeping of the information entered and its protection against unauthorised alteration are ensured.
19. Group II gas-hazardous work is performed without a permit to work. That type of gas-hazardous work covers periodically recurring gas-hazardous work that forms an integral part of the process (including sampling and the draining of apparatus), is characterised by similar conditions of performance, a constant location and nature of the work and a defined composition of the persons performing it. Such work is included in the list of gas-hazardous work. The safety measures for such work shall be set out in the process regulations, in the workplace operating instructions or in an instruction specially developed by the operating organisation with regard to the requirements of these Rules. Group II gas-hazardous work shall be recorded in the logbooks of gas-hazardous work carried out without a permit to work for gas-hazardous work (hereinafter - the logbook of gas-hazardous work) (a recommended form is given in Appendix No. 3 to these Rules). The logbook of gas-hazardous work shall be numbered, laced and sealed with a stamp (where one is held); its retention period is six months from the day it is completed. The logbook of gas-hazardous work may be kept in the form of an electronic document provided that the safekeeping of the information entered and its protection against unauthorised alteration are ensured.
20. A permit to work for gas-hazardous work is issued for each location and type of work to each work party carrying out such work and is valid for one shift. The composition of the work party performing the gas-hazardous work shall be specified in the permit to work. If the work has turned out to be unfinished while the conditions of its performance have not changed, which is confirmed by the results of analysis of the air environment, and the nature of the work has not changed, the permit to work for gas-hazardous work may be extended by the head of the structural subdivision, or by the person acting in his place, for the location of the gas-hazardous work, but by no more than one day shift. Where gas-hazardous work is performed as part of scheduled repair work at a facility, the permit to work for its performance is issued for the whole period of the repair work with daily extension by no more than one shift.
21. The head of the structural subdivision where the gas-hazardous work is to be carried out appoints the persons responsible for the preparation and conduct of the gas-hazardous work in accordance with the approved list. Where the person responsible for preparing the gas-hazardous work or the person responsible for carrying out the gas-hazardous work is replaced, a note is made in the permit to work. A specialist from among the engineering and technical personnel of the operating organisation (branch of the organisation) who has charge of the employees operating the facility (production unit, shop, installation, equipment), who is not engaged in operating the process for the period of that work and who knows the safe methods and techniques of gas-hazardous work is appointed the person responsible for preparing the gas-hazardous work. A specialist from among the engineering and technical personnel of the operating organisation (branch of the organisation) who is not engaged in operating the process for the period of that work and who knows the safe methods and techniques of gas-hazardous work is appointed the person responsible for carrying out the gas-hazardous work. Where gas-hazardous work is performed by employees of contractor organisations, a specialist from among the engineering and technical personnel of the contractor organisation who has charge of the persons performing the gas-hazardous work is appointed responsible for carrying it out, with mandatory supervision by an engineering and technical employee of the operating organisation (branch of the organisation). Where the preparation and the direct performance of the gas-hazardous work are carried out by the same set of persons, one manager may be appointed for its preparation and conduct provided that the person appointed knows the safe methods and techniques of the work and is released from other duties for the period of its performance.
22. The head of the structural subdivision determines the location and nature of the gas-hazardous work to be performed and, together with the persons responsible for the preparation and conduct of the gas-hazardous work, develops the actions for preparing the facility for the gas-hazardous work and the sequence in which they are performed, and also the actions ensuring the safe conduct of the work, determines the personal protective equipment, establishes the working regime (the duration of use of protective equipment and of breaks in the work) and the procedure for (frequency of) monitoring the air environment.
23. The permit to work for gas-hazardous work is signed by the head of the structural subdivision, or by the person acting in his place, at whose facility the gas-hazardous work is carried out, is agreed with the organisation's own PASS(F) certified to carry out gas rescue operations under the procedure established by the Regulations on the Certification of Emergency Rescue Services, or with a PASS(F) with which a service contract has been concluded, and is approved by the head of the operating organisation or an authorised deputy, or by the head of a branch or an authorised deputy. For facilities of trunk pipeline transport and underground gas storage facilities, agreement with the PASS(F) may be replaced by agreement with an official additionally appointed by an administrative document of the operating organisation and certified as to knowledge of these Rules, with notification of the PASS(F). The head of the operating organisation determines, by internal documents of the organisation, the structural subdivisions on which agreement of the permit to work for gas-hazardous work is conferred. A layout diagram of the location of the gas-hazardous work, signed by the head of the structural subdivision or by the person acting in his place, shall be attached to the permit to work for gas-hazardous work. Where work is carried out on shared service lines or at the boundaries of adjacent process facilities, at points where service lines and linear facilities of other structural subdivisions cross, the permit to work for gas-hazardous work is agreed with the heads of those structural subdivisions. A copy of the layout diagram of the location of the gas-hazardous work is passed to the heads of the structural subdivisions.
24. The permit to work for gas-hazardous work approved in two copies is held by the head of the structural subdivision, who passes one copy to the person responsible for preparing the gas-hazardous work for the performance of the preparatory work, while the second copy is passed to the PASS(F) or to another production service on which those powers are conferred in accordance with the documents of the operating organisation (branch of the organisation). Entries in the permit to work for gas-hazardous work shall be clear and easily legible. The permit to work for gas-hazardous work may be completed using a personal computer. Completion of the permit to work for gas-hazardous work in pencil shall not be permitted. Corrections in the text shall not be permitted, nor shall signatures of responsible persons made by means of a facsimile stamp, or photocopies of such signatures. The permit to work for gas-hazardous work may be issued and registered in the form of an electronic document. In that case the possibility of unauthorised alteration of the information in the permit to work shall be excluded, and conditions shall be ensured for keeping the permit to work for one year from the day it is closed. The possibility of using an electronic signature when agreeing and approving permits to work for gas-hazardous work is established by the internal documents of the operating organisation (branch of the organisation) in accordance with the requirements of Federal Law No. 63-FZ of 6 April 2011 'On Electronic Signature' (hereinafter - the Federal Law 'On Electronic Signature') (Collected Legislation of the Russian Federation, 2011, No. 15, Article 2036; 2016, No. 1, Article 65; No. 26, Article 3889).
25. When work is carried out in vessels, and also work involving the depressurisation of process equipment, pipelines and service lines, the diagram(s) of the arrangement of the shut-off valves, of the draining of product, flushing, purging and steaming and of the points where blanks are fitted, signed by the head of the structural subdivision or by the person acting in his place, shall be attached to the permit to work for gas-hazardous work.
26. Preparatory work means all types of work involved in preparing vessels, process equipment, pipelines and service lines for gas-hazardous work.
27. The facility is prepared for the gas-hazardous work to be carried out on it by the employees operating the facility, under the direction of the person responsible for the preparation.
28. To prepare the facility for gas-hazardous work, the whole set of preparatory work provided for in the workplace operating instructions or in documents specially developed by the operating organisation (standards, instructions, regulations, process flow charts) and in the permit to work for gas-hazardous work shall be performed. Measures shall be taken to reduce the degree of hazard of the gas-hazardous work by relieving the pressure from equipment operating under excess pressure, removing hazardous substances, preventing their ingress from adjacent process systems and eliminating possible sources of spark formation.
29. The location of the gas-hazardous work, within the area where vapours and gases of hazardous substances may enter, shall be marked out (fenced off), and the warning signs 'Gas' or 'Gas-hazardous work' shall be installed. At the decision of the person responsible for preparing the gas-hazardous work, sentries may additionally be posted to prevent unauthorised persons from entering the hazardous area.
30. Electric drives of moving machinery and other current-using equipment at the places where the work is carried out shall be disconnected from the power supply by a visible break (a break in the electrical circuit). A notice 'Do not switch on: personnel at work!' shall be displayed on the starting devices and is removed on completion of the work at the direction of the person responsible for carrying out the gas-hazardous work. Disconnection of the electric drive from the power supply (and its connection to it) shall be carried out by the electrical personnel of the operating organisation (branch of the organisation).
31. To assess the quality of the preparatory actions, before the gas-hazardous work starts a laboratory or rapid analysis of the air environment shall be carried out for the oxygen content (in enclosed premises, inside vessels (apparatus) and in excavation pits) and for the hazardous substances listed in the list of gas-hazardous work, according to the location and nature of the work, with the results recorded in the permit to work for gas-hazardous work.
32. During the preparation for gas-hazardous work, the availability and serviceability of personal protective equipment, tools, devices and the means of ensuring the safety of the persons carrying out the work are checked by the person responsible for the preparation of the gas-hazardous work. Before the start of the preparation of the facility for gas-hazardous work, the head of the structural unit at whose facility the gas-hazardous work is performed, or the person deputising for him, briefs the persons responsible for the preparation and performance of the gas-hazardous work on the specific features of the production process and on the characteristic hazards that may arise during the gas-hazardous work and upon which the work shall be stopped. The results of the briefing are recorded in the permit to work by the signature of the head of the structural unit or of the person deputising for him. The person responsible for carrying out the gas-hazardous work briefs the persons carrying out the work and checks their ability to use personal protective equipment, their knowledge of safe working methods and of the methods of giving first aid to casualties, with a note in the permit to work for gas-hazardous work. The person responsible for ensuring that the briefings are given is the head of the structural unit or the person deputising for him.
33. To ensure the safe performance of the preparatory work and of the gas-hazardous work itself, the following shall be done: employees engaged in the operation of the production process shall be warned of the gas-hazardous work being performed, with an entry in the process log (watch log, shift handover log); employees of contractor organisations shall be briefed on the principal hazardous factors of the production process.
34. On completion of the preparatory work, the person responsible for the preparation of the gas-hazardous work shall check the completeness and quality of the work performed and hand over the facility to the person responsible for carrying out the gas-hazardous work, with a signature in the permit to work.
35. Gas-hazardous work shall be commenced only after all the preparatory work and the actions provided for by the permit to work for gas-hazardous work and by the operating instructions have been completed. It shall not be permitted to increase the scope and nature of the work provided for by the permit to work for gas-hazardous work. The combination of gas-hazardous work and hot work in one room, and also in immediate proximity on an open area where fire- and explosion-hazardous substances may be released into the work zone, shall not be permitted.
36. For the safe performance of gas-hazardous work, the following shall be ensured: the sequence and mode of performance of the gas-hazardous work; monitoring of the condition of the air environment; the adoption of measures excluding the admission of persons not engaged in performing the gas-hazardous work to the site of that work.
37. Group I gas-hazardous work shall be performed by a work party of not fewer than two persons. The members of the work party shall be provided with the appropriate personal respiratory and skin protective equipment, protective clothing, protective footwear, tools, devices and ancillary materials.
38. Before the start of gas-hazardous work, each person carrying out the work is asked about his state of health. The engagement in gas-hazardous work of persons who have reported feeling unwell shall not be permitted.
39. Where the composition of the work party performing gas-hazardous work changes, the particulars of the members of the work party shall be entered in the permit to work for gas-hazardous work by the person responsible for carrying out the gas-hazardous work. The admission of employees newly included in the work party and authorised to perform gas-hazardous work is carried out in accordance with the requirements set out in paragraph 32 of these Rules.
40. A gas-hazardous location may be entered only with the permission of the person responsible for carrying out the work and in personal protective equipment put on outside the hazard zone.
41. Work shall begin in the presence of the person responsible for carrying out the work. The continuous presence of the person responsible for carrying out the work and of the representative of the PASS(F) at the work site, and the frequency with which they monitor the performance of the work, are determined by the permit to work for gas-hazardous work, having regard to the nature of the work performed.
42. When gas-hazardous work is performed during which explosive substances may be released into the work zone, the following shall be used: portable luminaires of explosion-protected design, matching in design the category and group of the explosive mixture; communication equipment of explosion-protected design; tools made of a material that excludes the possibility of sparking; footwear that excludes the possibility of sparking; respiratory protective equipment (according to the working conditions). Devices for connecting mobile and portable electrical equipment shall be located outside the explosion hazard zone. The period of continuous wearing of respiratory protective equipment by a worker is determined by the permit to work for gas-hazardous work, but shall not exceed thirty minutes.
43. Vessels (apparatus) to be opened, inspected, cleaned or repaired shall be freed of hazardous substances, disconnected from operating equipment, pipeline systems and utility lines by means of standard blanks in accordance with the diagram attached to the permit to work for gas-hazardous work and, depending on the properties of the hazardous substances they contained, washed, steamed, purged with inert gas and with air.
44. To assess the quality of the preparatory actions, an analysis of the air environment inside vessels (apparatus) for the content of oxygen and of hazardous substances (vapours, gases) shall be performed before the start of work inside them, with the results recorded in the permit to work.
45. Before the start of work, the warning notices “Gas” and “Gas-hazardous work” shall be posted on vessels (apparatus); they may be removed after completion of the work with the permission of the person responsible for carrying out the gas-hazardous work.
46. Where vessels (apparatus) are fitted with electrically driven stirring devices, the drive shall be disconnected from its power supplies by an isolating distance. The notices “Do not switch on: personnel at work!” shall be posted at the switchgear of the vessels (apparatus); they may be removed after completion of the work with the permission of the person responsible for carrying out the gas-hazardous work. The disconnection of the electric drive from the power supply, and its connection to the power supply, shall be performed by the electrical personnel of the operating organisation (a branch of the organisation).
47. Heated vessels (apparatus) shall be cooled to a temperature not exceeding thirty degrees Celsius before persons are admitted inside them. Where work has to be performed inside vessels (apparatus) above that temperature, additional safety measures shall be provided: continuous forced blowing with fresh air, the use of heat-protective suits and heat-insulating footwear, and breaks in the work. Work inside vessels (apparatus) at a temperature of fifty degrees Celsius and above shall not be permitted.
48. For the protection of the respiratory organs of employees inside vessels, hose-type breathing apparatus or oxygen self-contained breathing apparatus, or self-contained air breathing apparatus shall be used. The use of filtering breathing apparatus shall not be permitted.
49. Work inside vessels (apparatus) shall be performed by a work party of not fewer than two persons (the person working and the standby person). The person working inside the vessel (apparatus) shall use a safety belt or a safety harness with a signal and rescue line. When work is performed inside a vessel (apparatus), the person responsible for carrying out the gas-hazardous work shall be present at the work site.
50. Where it is necessary to increase the number of employees inside a vessel (apparatus), safety measures shall be developed, entered in the permit to work for gas-hazardous work and additionally implemented, providing for an increase in the number of standby persons (but not fewer than two standby persons), the procedure for entry and evacuation of employees, the arrangement of hoses, of the intake pipes of the breathing apparatus and of the signal and rescue lines, and the availability of communication and alarm equipment at the work site.
51. Where there is no visual contact between the person working and the standby person, a system of prearranged signals shall be established.
52. When work is performed inside a vessel (apparatus), the position of the person working and of the standby person during the work, and their actions in the event of situations involving a deterioration in the state of health of the person working or of the standby person, shall be determined in the permit to work for gas-hazardous work.
53. Work inside vessels (apparatus) without respiratory protective equipment may be carried out provided that the concentration of hazardous substances (vapours, gases) does not exceed the maximum permissible concentrations (hereinafter - MPC) in the air of the working area, that the oxygen content inside the vessels (apparatus) is not less than twenty per cent by volume and that the ingress of hazardous substances (vapours, gases) from outside is excluded, with an entry in the permit to work for gas-hazardous work and with the written permission of the person who approved the permit to work.
54. The actions ensuring the safety of work inside a vessel (apparatus) without personal respiratory protective equipment shall be set out in the operating instructions for workplaces or in instructions specially developed by the operating organisation (a branch of the organisation), having regard to the requirements of these Rules, and in the permit to work for gas-hazardous work, and include: a continuous guaranteed supply of fresh air into the vessel (apparatus) ensuring a normal air regime in the apparatus; continuous monitoring of the condition of the air environment; the availability of alarm and communication equipment (light, sound, radiotelephone) at the work site; the availability, for each person working inside the vessel (apparatus), of a safety belt or a safety harness with a signal and rescue line.
55. Work inside wells, collectors, tunnels and other similar locations without isolating respiratory protective equipment shall not be permitted.
56. For the descent of an employee into a vessel (apparatus), for work inside the vessel (apparatus) and for ascent from it, portable ladders made of non-sparking materials shall be used. The serviceability, stability and secure fixing of the ladder at the work site shall be checked in the presence of the person responsible for carrying out the gas-hazardous work.
57. All tools and materials needed for the work shall be passed into the vessel in a manner that excludes their falling and injuring employees.
58. Where circumstances arise that threaten the safety of an employee inside a vessel (signs of feeling unwell, an attempt to remove the mask of the breathing apparatus, breakage of the signal line, a fault in the hose, stoppage of the air blower), the work shall be stopped and the employee evacuated from the vessel.
59. Work in collectors, tunnels, wells, pits, trenches and similar structures shall be agreed with the heads of the structural units technologically connected with those facilities, who shall take measures excluding surge releases of harmful and of fire- and explosion-hazardous substances to the work site, and also with the heads of other structural units where the work is performed at points where common utility lines cross, with an entry in the permit to work for gas-hazardous work and with the attachment of a diagram of the work site within the boundaries of the areas (axes) of adjacent facilities of the structural units technologically connected with those facilities. A copy of that diagram shall be given to the heads of the structural units with which the permit to work for gas-hazardous work has been agreed. For the period of work in collectors, tunnels, wells, pits, trenches and similar structures, the sites of that work shall be fenced off and, during the hours of darkness, illuminated.
60. Where protective coatings are applied to the internal surfaces of vessels (apparatus) and the work is accompanied by the release of harmful and of fire- and explosion-hazardous substances, forced removal of those substances shall be provided for. The performance of such work without isolating respiratory protective equipment shall not be permitted.
61. On completion of work inside a vessel (apparatus), the person responsible for carrying out the gas-hazardous work shall, before the manholes are closed, satisfy himself that no employees, tools, materials or extraneous objects remain in the vessel (apparatus), then close the manholes and make an entry in the permit to work for gas-hazardous work on the completion of the gas-hazardous work. The person responsible for carrying out the gas-hazardous work shall notify the employees engaged in the operation of the production process of the completion of the gas-hazardous work and make an entry in the process log (watch log, shift handover log) and in the permit to work for gas-hazardous work.
62. Work on the installation (removal) of blanks classified as Group II gas-hazardous work is performed by operating personnel in accordance with the safety measures set out in the process regulations, in the operating instructions for workplaces or in an instruction specially developed by the operating organisation, having regard to the requirements of these Rules. The sequence of operations for the installation (removal) of blanks classified as Group I gas-hazardous work is specified in the permit to work for gas-hazardous work, with the attachment of a diagram of the installation of the blanks and having regard to the requirements of paragraph 18 of these Rules. The installation and removal of blanks shall be recorded in the register of installation and removal of blanks, stating their numbers and positions on the diagram of the installation of the blanks. The register of installation and removal of blanks may be kept in the form of an electronic document. In that case, the preservation of the information entered shall be ensured and the possibility of its unauthorised alteration excluded.
63. After the gas-hazardous work has been completed, the person responsible for carrying it out collects the second copy of the permit to work held by PASS(F) and, jointly with the head of the structural subdivision or the person deputising for him (the official responsible for the safe conduct of the process at the facility), they check that the gas-hazardous work has been performed in full and sign both copies of the permit to work, confirming that the work has been performed in full and that the permit to work for gas-hazardous work is closed.
64. After the permit to work for gas-hazardous work has been closed, the person responsible for carrying out the gas-hazardous work shall hand over to the head of the structural subdivision, or to the person deputising for him, one copy of the permit to work for gas-hazardous work and return the second copy to PASS(F) or to another production service upon which these powers are conferred in accordance with the documents of the operating organisation (branch of the organisation). Both copies of the permit to work for gas-hazardous work are kept for not less than six months from the date of its closure.
65. Responsibility for organising, developing and implementing measures to ensure safety during hot work at explosion and fire hazardous production facilities rests with the head of the operating organisation and with the officials appointed as responsible for fire safety by the head or by his authorised deputy.
66. Hot work comprises the heating of bitumen with an open flame, gas and electric welding, gas and electric cutting, petrol and kerosene cutting, work with a blowlamp, and the cutting of metal with power tools where sparks are produced.
67. Hot work at operating explosion and fire hazardous production facilities may be carried out where the work is urgent and there is no possibility of carrying it out at the permanent locations specially designated for that purpose.
68. Hot work shall be carried out only during daylight hours (the day working shift), except where possible accidents are being contained or eliminated in accordance with the plan of actions for containment and elimination of the consequences of accidents. For urgent work, urgent hot work may be carried out during the hours of darkness (the evening and night working shifts) and also on days off and public holidays, on the written permission of the person who approved the permit to work for hot work, in the presence of representatives of fire protection units or of an emergency rescue service (unit) certified for the right to carry out emergency rescue operations connected with fire extinguishing, under the procedure established by the Regulations on the Certification of Emergency Rescue Services, with notification of the persons who agreed the permit to work. The permit to work for hot work shall provide for additional actions to ensure that the work is carried out safely, taking account of the conditions under which it is performed during the hours of darkness, including: the availability of main and emergency lighting and the installation of additional lighting equipment (where necessary) of explosion-protected design; the provision, to personnel engaged in hot work during the hours of darkness, of personal lighting devices of explosion-protected design (personal torches or portable electric lamps with rechargeable batteries) and also of serviceable means of communication, equipment for monitoring the air environment and personal protective equipment appropriate to the nature of the possible hazard; limitation of the radius of scatter (fall) of sparks, scale and molten metal; performance of the work in the constant presence of the person responsible for carrying out the hot work.
69. Work to contain and eliminate the consequences of accidents is carried out without a permit to work for hot work, in accordance with the plan of actions for containment and elimination of the consequences of accidents, until the direct threat of harm to the life, health or property of other persons and to the environment has been removed.
70. Locations where hot work is carried out on a site containing explosion and fire hazardous production facilities are divided into permanent and temporary. Permanent locations are those in specially equipped rooms or on open areas where the formation of fire- and explosion-hazardous concentrations of vapours of hazardous substances is excluded (outside explosion hazardous zones), where hot work is carried out daily and where fire safety measures are provided for the whole period of its performance. The list of permanent hot work locations in the operating organisation (branch of the organisation) is determined by the organisational and administrative documents of its head. The list of permanent hot work locations states the position of each permanent hot work location by reference to the site or room of the facility, and also the types and quantity of first-aid fire-fighting equipment and the persons responsible for fire safety at those locations and for the safe performance of hot work. Permanent hot work locations shall be marked with a sign reading 'Hot work boundary'. Temporary locations are those where hot work is carried out periodically and is connected with the repair of equipment, pipelines, utility lines, buildings and structures, and also with the connection of completed construction projects to operating utility lines.
71. A permit to work for hot work is issued for each location and type of work to each work party carrying out that work and is valid for one shift. The composition of the work party performing the hot work shall be stated in the permit to work for hot work. If the work has not been completed and the conditions under which it is carried out have not changed, which shall be confirmed by the results of analysis of the air environment, the permit to work for hot work may be extended by the head of the structural subdivision at whose facility the hot work is carried out, or by the person deputising for him, but by not more than one day shift. Where hot work is carried out as part of scheduled repair work at a facility, the permit to work for that work is issued for the whole period of the repair work, with daily extension by not more than one working shift.
72. The head of the structural subdivision at whose facility hot work is to be carried out, or the person deputising for him, appoints the persons responsible for the preparation and performance of the hot work and determines the scope and content of the preparatory work and the sequence in which it is performed, the nature and content of the hot work, the procedure for monitoring the air environment and the personal protective equipment, and the measures to ensure fire safety at the work locations (organisational and technical fire safety measures).
73. The list of persons responsible for preparing the hot work location and of persons responsible for performing the hot work shall be approved by the head of the operating organisation or his authorised deputy, or by the head of the branch of the organisation or his authorised deputy. The list of persons responsible for the safe performance of hot work by a contractor organisation shall be approved by the head of the contractor organisation and sent to the operating organisation (branch of the organisation) at whose facility the work is to be carried out.
74. The person responsible for preparing the hot work location is a specialist appointed from among the engineering and technical personnel of the operating organisation (branch of the organisation) who has charge of the employees operating the facility, who is not engaged in running the process for the period of the preparatory work and who knows the conditions for preparing the facility for hot work. The person responsible for carrying out hot work (hereinafter, the person in charge of the work) is a specialist appointed from among the engineering and technical personnel of the operating organisation (branch of the organisation) who has been trained in the fire-technical minimum covering knowledge of the requirements of the Rules of the Fire Prevention Regime in the Russian Federation, approved by Government Resolution of the Russian Federation No. 1479 of 16 September 2020 (Collected Legislation of the Russian Federation, 2020, No. 39, Article 6056), and appointed as responsible for fire safety by the organisational and administrative documents of the head of the operating organisation (branch of the organisation) or of the person deputising for him. Where a contractor organisation carries out hot work at an operating explosion and fire hazardous facility (equipment, pipelines, utility lines, buildings and structures), a specialist from among the engineering and technical personnel of the contractor organisation who has charge of those performing the hot work shall be appointed as the person in charge of the work, with mandatory supervision by an engineering and technical employee of the operating organisation (branch of the organisation).
75. A permit to work for hot work is drawn up in two copies by the head of the operating organisation or by the person responsible for fire safety (a recommended form is set out in Appendix No. 4 to these Rules); the permit provides for the development and subsequent implementation of a set of actions for the preparation and safe performance of the work, and is handed over to the person responsible for preparing the hot work and to the person in charge of the work.
76. A diagram of the hot work location, signed by the head of the structural subdivision or by the person deputising for him, shall be attached to the permit to work for hot work.
77. Where hot work is carried out on shared utility lines or at the boundaries of adjacent process facilities, at points where utility lines and linear facilities cross, or at interconnected facilities of other structural subdivisions, the permit to work for hot work is agreed with the heads of those structural subdivisions. A copy of the diagram of the hot work location is handed over to the heads of the adjacent process facilities.
78. The diagram shall show the hot work location and the boundaries of the hazardous area, the air sampling points, the positions of shut-off valves and of blanks fitted on process equipment and pipelines, the positions of welding and other equipment for hot work, the positions of warning signs, the positions of vehicles and auxiliary equipment supporting the work, the positions of first-aid fire-fighting equipment and the escape routes. The hot work location shall be shown on the diagram by reference to existing facilities.
79. The permit to work for hot work shall be agreed with the fire protection service (where one exists) or with another service of the organisation upon which the internal documents confer the functions of ensuring fire safety measures, and also with the heads of the structural subdivisions technologically connected with the hot work location, and approved by the head of the operating organisation or by another official authorised by the head of the operating organisation. By the internal documents of the organisation, the head of the operating organisation determines the structural subdivisions upon which agreement of the permit to work for hot work is conferred and the officials who register permits to work for hot work in the register of permits to work. The register of permits to work for hot work may be kept as an electronic document where there are means ensuring that the information they contain is preserved and protected against alteration.
80. One copy of the registered permit to work for hot work remains with the person in charge of the work; the second is handed over by the person responsible for preparing the hot work to the fire protection service (where one exists) or to another service of the operating organisation upon which the functions of ensuring fire safety measures are conferred.
81. After the permit to work for hot work has been agreed and all preparatory actions have been completed, the person responsible for preparing the hot work and the person in charge of the work shall sign the permit to work for hot work. The possibility of performing the work is confirmed by the signature in the permit to work for hot work of a representative of the fire protection service or of another service of the operating organisation upon which the functions of ensuring fire safety measures are conferred. The head of the structural subdivision at whose facility the hot work is carried out, or the person deputising for him, having checked that the actions have been completed, authorises the hot work by signing the permit to work for hot work. Checking that the actions have been completed in full at hot work locations at remote hazardous production facilities is established by the internal documents of the operating organisation with the use of electronic means.
82. Entries in the permit to work for hot work shall be clear and legible. The permit to work for hot work may be completed using a personal computer. Completion of the permit to work in pencil shall not be permitted. Corrections to the text and signatures of responsible persons made by means of a facsimile stamp, and photocopies of them, shall not be permitted. A permit to work for hot work may be drawn up and registered as an electronic document. In that case the possibility of unauthorised alteration of the information in the permit to work shall be excluded and conditions ensured for storing the permit to work for one year from the date of its closure. The possibility of using an electronic signature when agreeing and approving permits to work for hot work is established by the internal documents of the operating organisation (branch of the organisation) in accordance with the requirements of the Federal Law 'On Electronic Signature'.
83. Preparatory work comprises all types of work connected with preparing equipment, utility lines and structures for hot work.
84. Preparation of a facility for hot work at that facility is carried out by the employees operating the facility under the direction of the person responsible for the preparation, including where the hot work is performed by contractor organisations.
85. When preparing for hot work, the head of the structural subdivision at whose facility the hot work is carried out, or the person deputising for him, jointly with the persons responsible for preparing and performing the hot work, determines the hazardous area, the boundaries of which are marked with warning signs and notices.
86. Welding, cutting and heating locations are marked with identification signs.
87. Apparatus, machinery, vessels, pipelines and other equipment on which hot work is to be carried out shall be shut down, freed of hazardous substances, disconnected from operating equipment, pipeline systems and utility lines by means of standard blanks in accordance with the diagram attached to the permit to work, and prepared for the hot work. The fitting and removal of blanks shall be recorded in the register of the fitting and removal of blanks, stating their numbers and positions on the blank installation diagram attached to the permit to work for hot work.
88. The electric drives of the moving mechanisms of apparatus, machinery and other equipment, and also other current-using equipment located in the hot work area, shall be disconnected from the power supply and disconnected from those mechanisms by a visible break. Notices reading 'Do not switch on: people at work!' shall be posted on starting devices; those notices are removed on completion of the work on the instruction of the person responsible for carrying out the hot work. Disconnection of the electric drive from the power supply, and its connection to the power supply, shall be carried out by the electrical personnel of the operating organisation (branch of the organisation).
89. The hot work location (platforms, metal structures and structural elements of buildings located in the hot work area) is cleared of combustible and fire- and explosion-hazardous substances. Where there are combustible structures in that area, they shall be protected against ignition by a metal screen, by blankets for isolating the seat of the fire or by non-combustible materials, and also doused with water.
90. In the hot work area the tightness of the closure of sewer manhole covers shall be checked. Manhole covers shall be covered with a layer of sand not less than ten centimetres thick within a steel or reinforced concrete ring.
91. Where hot work is carried out indoors, measures shall be provided to protect against the scatter of sparks and their entry into openings in the floor slabs between storeys and into troughs and pits where residues of combustible liquids, vapours and gases may accumulate. Indoors, in the hot work area, uninterrupted operation of the ventilation (supply and extract) and natural ventilation by opening transom windows and windows shall be ensured. Indoors, in the hot work area, measures shall be ensured to prevent sparks from entering extract ventilation systems.
92. Drain funnels, trough outlets and other devices connected with the sewerage system in which combustible gases and vapours may be present shall be sealed.
93. The hot work location shall be provided with a fire extinguisher and other first-aid fire-fighting equipment specified in the permit to work for hot work.
94. Hot work shall be started only after all preparatory work and measures to ensure fire safety at the work site provided for by the permit to work for hot work have been completed. It shall not be permitted to change the nature and content of the hot work provided for by the permit to work for hot work. It shall not be permitted to combine hot work and gas-hazardous work in one room, or in the immediate vicinity in an open area where fire- and explosion-hazardous substances may be released into the work zone.
95. Before hot work begins and after breaks of more than one hour, a sample shall be taken at the work site (in the working zone, apparatus, pipelines and service lines) for analysis of the air environment for the content of hazardous substances. Hot work shall not be permitted where fire- and explosion-hazardous substances are present in the zone of the work above twenty per cent by volume of the lower concentration limit of flame propagation.
96. While hot work is being performed, the condition of the air environment shall be monitored in the apparatus and service lines on which that work is carried out and in the zone of the hot work. The condition of the air environment shall be monitored in accordance with the permit to work for hot work.
97. Where, while hot work is being performed, the content of hazardous substances in the zone of the hot work exceeds the maximum permissible concentration (gas contamination), the hot work shall be stopped and resumed only after the causes of the gas contamination have been identified and eliminated. Hot work shall be started where hazardous substances are absent from the zone of the hot work or are present at a level not exceeding the maximum permissible concentration, which is to be confirmed by the results of monitoring of the condition of the air environment.
98. To ensure that hot work is performed safely, the head of the structural unit at whose facilities the hot work is performed shall warn the employees engaged in operating the process about the hot work being performed and make an entry about that work in the process log (watch log, shift handover log). Throughout the whole time the hot work is performed, the employees engaged in operating the process shall take measures ruling out the release of hazardous substances, including fire- and explosion-hazardous substances (vapours, gases), into the air environment. It shall not be permitted to open hatches and covers of apparatus containing hazardous substances, to perform process operations involving the unloading, transfer and draining of products, to load products through open hatches or to perform other operations that may lead to gas contamination or dust contamination of the places where the hot work is carried out.
99. Before the preparation and performance of hot work begins, the head of the structural unit, or the person acting in that capacity, at whose facility the hot work is carried out briefs the person responsible for preparing the hot work and the person in charge of the work on fire safety and industrial safety measures during the work at that facility. The person in charge of the work briefs the workers carrying out the work and also familiarises them with the scope of the hot work at the site. The briefing is recorded in the permit to work for hot work by the signatures of the workers carrying out the work and of the person in charge of the work. Before the hot work begins, each worker carrying out the work is asked about their state of health. Persons who have reported feeling unwell shall not be permitted to take part in hot work.
100. A change in the composition of the work party shall be recorded by an entry in the permit to work. The person in charge of the work shall brief the workers newly included in the work party in accordance with the requirements established in paragraph 99 of these Rules. Where the person responsible for preparing the hot work or the person in charge of the work is replaced, a corresponding note is made in the permit to work for hot work.
101. Daily admission to hot work is granted by the head of the structural unit of the facility, or by the person acting in that capacity, and is confirmed by a signature in the permit to work.
102. To ensure that hot work is performed safely, the following shall be checked: the serviceability and completeness of the welding and other equipment for hot work; the availability and serviceability of primary fire-extinguishing equipment; the availability of protective clothing, protective footwear and protective shields and their suitability for the conditions of the work; the personal protective equipment provided for by the permit to work for hot work.
103. Hot work shall be started and performed in the presence of the person in charge of the work, who monitors the work of the workers carrying it out. Persons not engaged in performing the work shall not be permitted in the zone of the hot work.
104. After the hot work is completed, the person in charge of the work shall check that the work has been performed in full and arrange for the workplaces to be tidied. Tools, equipment and materials shall be removed from the hot work site, and the workers who performed the hot work led out of it. The permit to work for hot work shall be closed. The person in charge of the work shall notify the employees engaged in operating the process that the hot work is completed and make an entry to that effect in the process log (watch log, shift handover log).
105. After the hot work is completed, the person in charge of the work collects the second copy of the permit to work held by the fire protection service (where one exists) or by another service of the operating organisation charged with fire safety functions and, together with the head of the structural unit or the person acting in that capacity (the person responsible for the safe operation of the process at the facility), inspects the hot work site and the completeness of the work and, to rule out any possibility of ignition, arranges for the place of a possible seat of fire to be monitored (kept under observation) for four hours by the employees of the structural unit engaged in operating the process, after which they sign both copies of the permit to work, confirming that the hot work is finished and that the permit to work is closed. After the permit to work for hot work is closed, the person in charge of the work shall hand one copy of the permit to work for hot work to the head of the structural unit or the person acting in that capacity and return the second copy to the fire protection service (where one exists) or to another production service charged with fire safety functions under the documents of the operating organisation (branch of the organisation). Both copies of the permit to work for hot work are kept for not less than six months from the date of its closure.
106. For hot work inside a vessel (apparatus), a permit to work for gas-hazardous work shall be issued in addition to the permit to work for hot work in accordance with Chapter II of these Rules (the registered numbers of the permits to work are stated in the permits to work for hot work and for gas-hazardous work). Hot work in vessels (apparatus) shall be carried out with the hatches fully open and with forced air exchange providing a normal air condition in the zone of the hot work.
107. Repair work at the hazardous production facilities specified in paragraph 2 of these Rules means the set of work to restore the serviceability or operability of facilities and to restore the service life of technical devices (facilities) and of their component parts, including gas-hazardous work, hot work and earthworks. The provisions of this section apply to hazardous production facilities of chemical, petrochemical and oil and gas processing production.
108. Repair work is divided into the following types: planned repair work; unplanned repair work; emergency recovery repair work. Planned repair work means work performed in accordance with the design documentation for the facility, the regulatory technical documents of the manufacturer of the technical devices and the technical documentation of the operating organisation, including operation-by-operation quality control of the repair work, in particular by methods of technical diagnostics, and integrated and individual testing. Unplanned repair work means work connected with equipment failure or with the emergency technical condition of equipment. Emergency recovery repair work means work performed to the minimum extent necessary to restore the safe operability of the facility.
109. Repair work is carried out in two stages: the first stage is preparatory work; the second stage is the repair work itself.
110. The basis for shutting down a facility for repair on the territory of an operating production unit is an organisational and administrative document of the head of the operating organisation (branch of the organisation) or of an authorised deputy. The basis for shutting down individual items of equipment, technical devices and service lines for repair in the operating production of a structural unit is an organisational and administrative document of the head of the structural unit or of the person acting in that capacity. The organisational and administrative document shall state the person in direct charge of the work (where the work is performed by a contractor organisation, a representative of that organisation), and also the persons responsible for preparing the facility as a whole, or the equipment, technical devices and service lines, for repair, chosen from among the specialists in charge of the employees who operate the facility or the equipment, and responsible for the actions needed to ensure the safety of the work during the repair and for operational communication with the contractor organisation. The organisational and administrative document shall also state the time limits for shutdown, preparation, repair and return of the facility, or of the equipment, technical devices and service lines, to operation.
111. At a facility where repair work is performed by several contractor organisations and by the client, overall coordination of the repair work is carried out by a person appointed by the head of the operating organisation (branch of the organisation) or by an authorised deputy.
112. For planned shutdown repair work at a facility carried out under an order, the operating organisation (the client) transfers technical documentation to the contractor organisation before the repair begins, including: the defects list; the estimate documentation; the list of equipment, pipelines and service lines subject to increased safety requirements in operation; the design or as-built documentation for the facility (drawings, diagrams); the design documentation for the facility being repaired; the technical specifications for the repair of the main equipment, technical devices and assemblies. Before the repair work begins, the operating organisation shall: draw up a preparatory work plan; arrange for the manufacture of the assemblies and parts needed for replacement; purchase the equipment, valves and fittings, spare parts, pipes and materials required under the defects list. Before the repair work begins, the contractor organisation shall develop a work execution plan and a network (linear) schedule for complex and labour-intensive repairs. The work execution plan shall be agreed with the representatives of the operating organisation (branch of the organisation) appointed by internal documents of the operating organisation (branch of the organisation) and signed by the person in direct charge of the work of the contractor organisation. The person in direct charge of the work of the contractor organisation shall familiarise all the workers carrying out the work with the work execution plan against signature. For emergency recovery work requiring a prior technical survey, the operating organisation (branch of the organisation) shall develop and transfer to the contractor organisation a work execution plan for the emergency recovery work.
113. The preparatory work plan shall define the preparation of the process facility or equipment, process pipelines and service lines for the repair work and establish the boundaries of the repair area of the facility being repaired. The preparatory work plan is drawn up by the operating organisation (branch of the organisation) in accordance with the requirements of the process regulations and the operating and repair instructions and covers the sequence of work to free the facility or equipment of hazardous substances, to disconnect it from operating equipment, pipeline systems and service lines and, depending on the properties of the hazardous substances they contained, work on washing, steaming and purging with inert gas and air. The preparatory work plan is signed by the person responsible for preparing the facility for repair. The work execution plan shall be developed for carrying out the repair work and define the work technologies, the quality of their performance, the specific places where the work is performed, the machinery and devices used, the number of workers, the protective equipment, the calendar time limits for performance, the resources and the actions for the safe performance of the work.
114. A contractor organisation may use stationary lifting appliances, including those controlled from the floor by means of a push-button control unit suspended from the crane or from a stationary control desk, and may connect to operating networks of electric power, compressed air, steam and water (hereinafter - energy supply networks) for repair work with the permission of the operating organisation (branch of the organisation), upon application by the person in direct charge of the work of the contractor organisation and with a corresponding entry in the permit to work for repair work. Connection to and disconnection from the energy supply networks is carried out by the operating organisation (branch of the organisation). Connection to the electrical networks of the mobile electrical equipment of the contractor organisation for repair work, stating the points of connection, and its disconnection after the work is completed are carried out by the electrical personnel of the operating organisation (branch of the organisation) with an entry in the operating log.
115. A permit to work for repair work is issued by the operating organisation (branch of the organisation) (a recommended form is given in Appendix No. 5 to these Rules), in accordance with the organisational and administrative documents of the operating organisation (branch of the organisation), for a defined scope of work stating the facility being repaired in the allocated repair area, and is valid for the whole time needed to perform that scope of repair work by one and the same repair work party, with daily confirmation that the repair work may proceed, given by the persons responsible for preparing and performing the repair work and by their signatures in the permit to work for repair work. Permits to work shall be registered in the register of permits to work for repair work and given the next number in sequence. The procedure for registration and the form of the register of permits to work for repair work are determined by an internal document of the operating organisation (branch of the organisation). The retention period for the register of permits to work for repair work is not less than six months from the date it is completed. The register of permits to work for repair work may be kept as an electronic document. In that case the information entered shall be preserved and the possibility of its unauthorised modification ruled out.
116. The person in direct charge of the work of the contractor organisation shall be a specialist from among the engineering and technical personnel, appointed by an order (instruction) of the head of the contractor organisation. The person in direct charge of the work shall know the possible hazards and the way they manifest themselves during repair work, and the safe methods and techniques of repair work, and personally monitor the progress of the repair work.
117. A permit to work for repair work is issued in two copies. The first copy of the permit to work for repair work, signed by the head of the structural unit of the facility being repaired, is given to the person in direct charge of the work of the contractor organisation; the second copy is held by the head of the structural unit of the facility being repaired, who is responsible for admitting the repair work parties of the contractor organisation to the repair work.
118. The head of the structural unit of the facility being repaired and the person in direct charge of the work of the contractor organisation jointly determine the technical and organisational actions ensuring the safety of the repair work.
119. After all the actions provided for by the preparatory work plan and by the permit to work for repair work have been performed, the person responsible for preparing and handing over the facility for repair and the person in direct charge of the work of the contractor organisation, together with the head of the structural unit of the facility being repaired, check that the actions have been performed in full, draw up a certificate of handover and acceptance of the facility for repair in the form established by internal documents of the operating organisation (branch of the organisation), and sign the permit to work for repair work. Without a certificate of handover and acceptance of the facility for repair, a permit to work for repair work is not issued.
120. Daily admission of the repair work parties of the contractor organisation to the repair work, with extension of the permit to work, is confirmed by the person responsible for preparing the facility, by the person in direct charge of the work of the contractor organisation and by the head of the structural unit of the facility being repaired through their signatures in the permit to work for repair work.
121. Before the repair work begins, the head of the structural unit of the facility being repaired, or the person acting in that capacity, shall brief the person in direct charge of the repair work and the workers carrying out the repair work of the contractor organisation on fire safety and industrial safety measures and on the possible hazardous and harmful production factors typical of the hazardous production facility in the allocated repair area. The briefing is recorded in the briefing log of the structural unit by the signatures of the workers carrying out the repair work. A note about the briefing (date, signature) is made in the permit to work for repair work.
122. The workers carrying out the repair work are briefed on the safety measures to be observed during that work by the person in direct charge of the work of the contractor organisation, with signatures in the permit to work for repair work. The composition of the workers carrying out the repair work, with a note that the briefing has been given, is recorded in the permit to work for repair work.
123. A change in the composition of the work party carrying out the repair work shall be recorded in the permit to work for repair work by the person in direct charge of the work of the contractor organisation, with a briefing given to the newly included workers.
124. Conditions ruling out the possible appearance of fire- and explosion-hazardous and toxic substances shall be created in the repair area. Where repair work is performed on process equipment from which hazardous substances may be released into the repair area, the condition of the air environment shall be analysed, the results of the analysis shall be entered in the permit to work for repair work, and the procedure and frequency of monitoring of the air environment in the repair area determined. The analysis of the condition of the air environment in the repair area shall be carried out at the request of the person in direct charge of the work of the contractor organisation. Repair work shall be carried out where hazardous substances are absent from the repair area.
125. A permit to work for repair work shall be re-issued, and the repair work shall be suspended, where: the measures ensuring the safety of the work have been breached; the scope and nature of the work have been changed so that the isolation schemes and the working conditions change; part of the equipment under repair, of the process unit, of the pipeline sections or of the utility lines has been put into operation (unless this relates to the testing or trial operation of that equipment or of those pipeline sections); an accident at work has occurred involving a person performing the repair work; the immediate supervisor of the work of the contractor organisation has been replaced.
126. Where, during planned (unplanned) repair work on technical devices and process equipment that have been shut down and prepared for repair, gas-hazardous and (or) hot work has to be performed, permits to work are drawn up by the operating organisation (a branch of the organisation) both for the repair work and for the gas-hazardous and hot work in accordance with the requirements of Chapters II and III of these Rules. The registration numbers of the permits to work for gas-hazardous and hot work are stated in the permit to work for repair work. The permits to work drawn up for gas-hazardous and (or) hot work are attached to the permit to work for repair work. The permit to work for gas-hazardous and (or) hot work is issued for the whole period of the repair work, determined by an administrative document of the head of the operating organisation (a branch of the organisation) or of his authorised deputy in accordance with the requirements of paragraph 110 of these Rules, and is extended daily for no more than one work shift, provided that the conditions and the nature of the work have not changed. Where the conditions or the nature of the work change, the permit to work for hot or gas-hazardous work opened earlier is closed and a new one is drawn up.
127. Where earthworks are carried out in the repair area or on the site of an operating production facility, the operating organisation (a branch of the organisation) shall provide the contractor organisation with a permit to work for earthworks, agreed with those structural units of the operating organisation (a branch of the organisation) which are charged with agreeing the permit to work for earthworks by the internal documents of the operating organisation (a branch of the organisation). The permit to work for earthworks states the place and the nature of the work (by hand or using machinery), the times at which the work starts and finishes, the conditions for the safe performance of the work, the composition of the work party and the persons responsible for preparing and carrying out the earthworks. A plan showing the exact boundaries of the earthworks and the underground structures and utility lines within those boundaries is attached to the permit to work for earthworks. The boundaries of the earthworks shall be marked on site by the operating organisation (a branch of the organisation) with marker signs.
128. After the repair work has been completed, the immediate supervisor of the work of the contractor organisation, together with the head of the structural unit of the facility under repair, checks the completeness of the repair work to ensure that the facility is safely put into operation, draws up a certificate of handover and acceptance of the facility into operation in the form established by the internal documents of the operating organisation (a branch of the organisation), closes the permit to work for repair work and signs it. Without a certificate of handover and acceptance of the facility into operation, the permit to work for repair work may not be closed.
129. A permit to work for repair work shall be kept by the contractor organisation and by the operating organisation for six months from the day on which the certificate of handover and acceptance of the facility into operation is signed.
130. Entries in a permit to work for repair work shall be clear and legible. A permit to work may be completed using a personal computer. Completion of a permit to work in pencil shall not be permitted. Corrections to the text, signatures of responsible persons made by facsimile stamp and photocopies of such signatures shall not be permitted. A permit to work for repair work may be drawn up and registered as an electronic document. In that case the possibility of unauthorised alteration of the information in the permit to work shall be excluded, and conditions shall be ensured for keeping the permit to work for one year from the day on which it is closed. Whether an electronic signature may be used when permits to work for repair work are agreed and approved is established by the internal documents of the operating organisation (a branch of the organisation) in accordance with the requirements of the Federal Law "On Electronic Signature".
131. Preparatory work for the repair of a facility is carried out in accordance with the plan of preparatory work drawn up for it. All work to prepare the facility for repair is performed by the employees operating the facility, under the direction of the person responsible for preparing and handing over the facility for repair, in accordance with the requirements of the process regulations and of the instructions on operation and on the safe performance of repair work, including preparatory work for gas-hazardous and hot work in accordance with the requirements of Chapters II and III of these Rules (where such work has to be carried out).
132. Once the preparatory work has been completed, the completeness and quality of its performance shall be checked, the air in the process facility or equipment, in the process pipelines and in the utility lines shall be analysed for oxygen content and for hazardous substances (vapours, gases), and the handover of the facility for repair shall then begin.
133. The places where repair work is carried out shall be marked out (fenced off) and warning signs shall be installed. Lighting shall be provided at night-time.
134. Actions for the safe performance of repair work are carried out in accordance with the work execution plan drawn up for them.
135. Starting repair work during shutdown repairs without a permit to work for repair work drawn up for it shall not be permitted; where gas-hazardous and hot work has to be carried out, it shall not be permitted without the permits to work drawn up for such work. Gas-hazardous work shall not be permitted at night-time, except for urgent work carried out in compliance with the requirements of paragraph 18 of these Rules. Hot work shall not be permitted at night-time inside process equipment and tanks, in wells and tunnels, or in sumps and trenches more than one metre deep. During planned shutdown repair work, hot work may be carried out during the hours of darkness (the evening and night work shifts) by separate work parties on an area (site) set aside within the repair area, access to which is restricted by solid enclosing structures made of precast reinforced concrete, metal or other fire-resistant materials not less than 2.2 metres high, provided that the requirements of Chapter III of these Rules are complied with. Where there is a justified need to carry out repair work at the facility during the hours of darkness, additional safety measures shall be provided, including: main and emergency lighting, and the installation of additional lighting equipment (where necessary) of explosion-protected design; the provision to the personnel engaged in hot work during the hours of darkness of individual lighting devices of explosion-protected design (personal torches or portable battery-powered electric lamps), and of serviceable means of communication, equipment for monitoring the air and personal protective equipment appropriate to the nature of the possible hazard; the performance of the work in the constant presence of the person responsible for carrying out the hot work.
136. Employees operating a facility, equipment, technical devices or utility lines shall not be permitted to carry out any work on them after they have been handed over for repair under a certificate of handover and acceptance of the facility for repair.
137. Persons performing repair work shall carry out the work only at the places specified in the permit to work for repair work. Persons performing repair work shall not be permitted to be present at other facilities under repair.
138. Using the metal and reinforced concrete structures of buildings and installations as supports when lifting and moving equipment and parts (assemblies) of it, without verifying the strength of those structures by calculation, shall not be permitted.
139. Where the situation in the repair area changes (vapour emissions appear, or chemical or fire- and explosion-hazardous substances leak), or where it changes at an operating facility located nearby, the persons performing the repair work shall immediately stop work, move to a safe place and report to the immediate supervisor of the work or to the person responsible for preparing the facility for repair. Work may be resumed only after the causes of the change in the situation in the repair area have been established and eliminated, with the permission of the immediate supervisor of the work and of the person responsible for preparing the facility for repair, and also of the head of the structural unit of the facility under repair.
140. When repair work is carried out, roads, driveways and approaches to sources of external fire-fighting water supply, and to the site of the facility under repair, shall not be obstructed by equipment, materials or scrap metal.
141. Earthworks on the site of operating production facilities comprise work involving the excavation of soil (the digging of pits and trenches).
142. Before earthworks begin, and before the soil is excavated, temporary protective barriers and warning signs shall be installed around the whole perimeter of the place where the work is carried out, to prevent unauthorised persons and vehicles from chancing into the repair area, whose actions could cause an accident and (or) their injury at the facility.
143. Before earthworks begin, the persons performing the earthworks shall undergo a briefing at the workplace on fire safety and industrial safety measures and on the hazardous and harmful production factors that may be present at the places where the work is carried out.
144. Actions for the safe performance of earthworks are carried out in accordance with the work execution plan drawn up for them, taking account of the requirements of the legislation of the Russian Federation on town-planning activity. The work execution plan defines the technology of the work, the specific places where the work is performed, the machinery and appliances used, the number of workers, the protective equipment, the calendar deadlines for performance, the resources and the actions for the safe performance of the work. When earthworks are performed, actions shall be provided to prevent hazardous production factors affecting the safe performance of the work from arising. Earthworks on the site of operating production facilities are carried out in the presence of the person responsible for them.
145. When pits and trenches are dug more than one metre deep, measures shall be taken to prevent the soil from overhanging and collapsing (the forming of slopes, the shoring of walls).
146. Where the situation in the area of the earthworks changes (vapour emissions appear, or chemical or fire- and explosion-hazardous substances leak), the work shall be stopped immediately and the persons performing it removed from the hazardous places. Work shall be resumed after the causes of the hazardous production factors affecting the safe performance of the work have been established and eliminated, and after the air has been monitored to confirm that no hazardous substances are present in the area of the earthworks.
147. Where underground structures and utility lines not shown on the plan are discovered, the work shall be stopped immediately and continued only with the permission of the person responsible for it and of the owner of the underground structures and utility lines, with additional measures for the safe performance of the work being drawn up.
148. Open pits and trenches shall be fenced off and, where necessary, provided with crossing bridges, and shall be lit during the hours of darkness. Where vehicles pass, warning (prohibitory) road signs shall be installed, and signal lighting shall be provided during the hours of darkness.
149. After the earthworks have been completed, the workplace shall be cleaned, the pit or trench shall be backfilled, and the site shall be restored to its original condition. Tools, tackle and appliances used in the work, protective clothing, protective footwear and personal protective equipment, once cleaned of soil, are put away in the place designated for their storage.
150. After the repair work has been completed, the facility shall be tested in accordance with the requirements of the technical documentation of the organisations that manufactured the equipment and the technical devices and of the instructions of the operating organisation (a branch of the organisation). Where the test results are positive, the quality of the repair of the facility is assessed and the facility is put into operation.
151. After the repair work has been completed (before the facility is started up), the contractor organisation performing the work transfers to the operating organisation (a branch of the organisation) all the as-built documentation evidencing the quality of the repair work performed that is required by the technical specifications for the performance and acceptance of the work.
152. A facility whose repair has been completed shall be accepted into operation under a handover and acceptance certificate and shall be permitted for operation after checks of: the assembly of the process system; the removal of blanks; the leak-tightness testing of the systems; the operability of the monitoring, alarm, control and emergency protection systems; the effectiveness and response time of the inter-unit shut-off (isolating) devices; the presence and serviceable condition of flame arresting devices and safety devices; the serviceable condition and required effectiveness of the ventilation systems; and the conformity of the installed electrical equipment with the requirements of the regulatory technical documents on the construction of electrical installations. The completeness and quality of the as-built repair documentation, the condition of the site of the facility and of the workplaces, and the readiness of the employees operating the facility to perform their principal duties shall be checked.
